That’s why we’re excited to announce our new partnership with PocketRN, a national leader in virtual nursing support, designed to bring a higher level of dementia care to the families we serve across the South Shore and MetroWest.
What This Partnership Means for Families
Caring for someone with Alzheimer’s disease or another form of dementia is one of the most demanding and emotionally complex journeys a family can face. The days are long, the decisions are hard, and the need for reliable support can feel urgent at every turn. This partnership was built with exactly those families in mind.
Through our collaboration with PocketRN, eligible families will now have access to two powerful layers of support working together: PocketRN’s expert registered nurses through the CMS GUIDE (Guiding an Improved Dementia Experience) program, and North River Home Care’s trusted in-home respite care — up to 72 hours of no-cost care for qualified clients. Together, these services help families stay safely at home, reduce caregiver burnout, and feel genuinely supported rather than stretched thin.
A Nurse for Life — and a Team You Can Count On
PocketRN’s “Nurse for Life” model is built around continuity and connection. Families receive ongoing education, care navigation guidance, and 24/7 access to virtual nursing support, so there’s always someone knowledgeable available when questions come up, day or night.
Paired with North River Home Care’s relationship-centered, hands-on in-home care services, this means families receive both the clinical expertise and the personal, day-to-day support that can make all the difference. It’s comprehensive care that serves not just the person living with dementia, but for the entire family unit.

Who is Eligible?
The program is available to individuals enrolled in Original Medicare Parts A and B who have a diagnosis of dementia or Alzheimer’s disease, are not on hospice, and are not at a skilled nursing facility.
