Quick answer: Hourly vs 24 hour home care comes down to when your loved one needs help. Choose hourly care if your loved one is mostly steady and needs help with a few daily tasks. Choose overnight care if nights are the hard part. Choose 24-hour care if your loved one can’t be left alone at any time. You can start small and add hours as needs change.
You’re not sure how much help your mom or dad needs. That’s normal. Most families start with a guess and adjust.
You’re also not alone in wanting home to work. In AARP’s 2024 survey, 75% of adults 50 and older said they wish to stay in their current homes as they age. The right schedule is what makes that possible.
This guide covers the three common schedules: hourly, overnight and 24-hour care. It shows what each one covers and how to tell which fits. It also shares what care costs, so the numbers don’t surprise you.
What is hourly home care?
Hourly home care is a caregiver who comes for a set shift to help with daily tasks. At North River, our shifts start at four hours. Hourly care is part of our companionship and personal care service. It often includes:
- Bathing, dressing and getting to the bathroom
- Meals and light housekeeping
- Reminders for medicine your loved one already takes
- Rides, errands and company
Hourly care often fits if your loved one can be alone for part of the day, but mornings or evenings are rough. It also fits a spouse who needs a few hours to rest or run errands. If you need a longer break, read about respite care.
What is overnight care?
Overnight care is a caregiver in the home through the night. Our overnight caregivers stay awake, so someone is ready if your loved one needs help.
Overnight care often fits if:
- Your loved one wakes up confused or tries to leave the house at night.
- They’ve fallen on the way to the bathroom.
- A family caregiver is worn out from broken sleep.
Getting the night covered can change how the whole week feels.
What is 24-hour care?
24-hour care means a caregiver, or a team that takes turns, is there around the clock. A person is awake and ready at all hours. If a caregiver is sick, we send cover so your loved one is never left alone.
24-hour care often fits if:
- Your loved one can’t be left alone at any time.
- They’ve just come home from the hospital or rehab. Our first-week checklist can help.
- Dementia has reached the point where safety is the main concern.

A simple way to start
Picture a mom who is steady during the day but wakes up confused at night. Her family might start with overnight care a few nights a week. If mornings get harder, they can add hourly shifts. If she can no longer be alone, they can move to 24-hour care. The plan follows what she needs.
How to choose: five questions
- When is the hardest part of the day or night? Cover that time first.
- Is it safe to be alone for an hour? For a night? If the honest answer is no, plan for more hours.
- Has anything changed lately? A fall, a hospital stay or a new diagnosis often means more help for a while.
- How are you doing? If you’re the family caregiver and you’re worn out, that counts. Your rest is part of the plan.
- What would make the week easier for everyone? Start there.
When home may not be the safest place
We’ll tell you if we think your loved one needs more than home care can offer. Sometimes a different setting is the right call. Our guide to home care vs. a nursing home can help you think it through. We’d rather say so than sell you hours you don’t need.
