What Is a Geriatric Care Manager?

A Geriatric Care Manager, more frequently referred to now as a Care Manager, is an expert who helps families navigate the many decisions that come with caring for an aging loved one. Often with a background in nursing, social work, and/or gerontology, a Geriatric Care Manager conducts a comprehensive assessment of a person’s needs and creates a practical plan of care that can evolve over time.

Their work often includes coordinating and communicating with doctors and other healthcare providers, reviewing medications, arranging medical equipment, advocating during hospital and rehabilitation transitions, and helping families understand insurance and benefits. They also recommend trusted resources, from home care to legal and financial professionals, and serve as a steady point of coordination so families do not have to piece everything together on their own.

Katherine’s Story

Katherine, an 83-year-old retired schoolteacher, had lived in her Norwell home for more than four decades. Widowed several years ago, she treasures her independence and the comfort of familiar surroundings, from her garden to her Sunday phone calls with her two adult children.

Over the past year, however, her Parkinson’s disease had begun to progress, and a recent fall sent her to the hospital and then to a short rehabilitation stay.
